to your insurance company.

Student Health Insurance
Ohio State ATI students enrolled for 6 or more credit hours will automatically be enrolled in the university-sponsored Student Health Insurance Plan (SHIP). ATI students with other coverage, such as that provided by an employer, parent's employer, or spouse's employer, may waive out of the Student Health Insurance Plan when they register for classes by completing the on-line waiver form. If the insurance waiver form is not completed fully by the 15th day of the quarter, students will be enrolled in SHIP and billed accordingly.
Ohio State ATI students who do not currently have health insurance coverage and want to waive out of the University-sponsored Student Health Insurance Plan (SHIP) can stop in the Academic Affairs Office, 219 Halterman Hall, prior to the 15th day of the quarter to sign an insurance waiver request form.
All eligible students (6 or more credit hours) who enroll in the Student Health Insurance Plan in the autumn term are automatically enrolled for annual coverage, which includes the autumn, winter and spring term coverage, as long as they maintain eligibility. All eligible students who initially enroll in the winter term are automatically enrolled for winter and spring terms if they maintain their eligibility; and all eligible students who initially enroll in the spring term, are automatically enrolled for only the spring term. Students are not required to enroll for the summer term (if it is an off-term), but should consider purchasing it in order to maintain continuity of coverage.
If a student wishes to change their coverage status or level of coverage after their initial selection for the academic year, they can only do that if they experience a qualifying event. A qualifying event is defined as an event that could result in a change of coverage status and includes: marriage, divorce, involuntary loss or involuntary gain of coverage from another health plan, dependent reaching the age limit of another health insurance plan, or first time arrival of dependents(s) to the United States from a foreign homeland.
To file a coverage status change form, please see the “Important Forms” section on the OSU Student Health Insurance main page at http://shi.osu.edu/index.asp The coverage status change form must be filed with Koster Insurance (Phone:(800) 254-2461) within 31 days of the qualifying event.
SMART
The Ohio State ATI Substance Management Awareness and Resource Team
(SMART) meets periodically to address the use and abuse of alcohol
and other drugs on the campus. Students are encouraged to get involved
in various activities.

Law Enforcement
The University Police are the responsible agency for all investigations
and reports involving incidents on the Wooster campus. This includes
all housing, Ohio State ATI, OARDC and outlying Ohio State University
operations.
What can you do?
Keep your car locked and valuables out of sight.
Don't leave purse, wallet, etc. lying around.
Use common sense - Crime can occur anywhere, Ohio State ATI is
NOT exempt!
All students should have received a campus safety brochure, which contains
more details on the department, crime statistics, and programming. If
you did not receive a copy or you would like further information, please

Traffic and Parking
NO parking permit is required for the Wooster campus.
Park only in marked spaces within a parking lot.
Report all traffic accidents to the police.
Driver's license and Ohio State University driving permit is
required for driving state vehicles.
All state traffic laws apply to campus roadways.
Zero tolerance for driving under the influence of alcohol/drugs.
Fire Prevention and Safety
Evacuate buildings when alarm sounds.
Assemble at designated location.
Tampering with safety devices is prohibited by law.
Report work related injuries to ATI Business Office.

Special Note on Alcoholic
Beverages and Illegal Drugs
NO person may consume an alcoholic beverage in a public place.
NO person under 21 may possess, purchase, consume, etc. any alcoholic
beverage.
NO person may provide, purchase, etc. any alcoholic beverage
to a person under 21. This includes hosting parties.
Under 21 and drink and drive? .02% blood alcohol (1 beer) makes
you DUI.
Most crimes on campus involve alcohol victims and suspects.
Random drug sweeps are conducted on campus, including housing
areas.
Violations will result in appropriate University and criminal
court action.
The Wooster Campus Ohio State University Police mission is to help provide
a safe environment in which the academic and research missions of Ohio
State ATI and OARDC may be pursued. This cannot be accomplished solely
by the police. It is a cooperative effort of the campus community and
the police working together
for a safer campus.
